REQUEST FOR QUALIFICATIONS FOR CONSTRUCTION MANAGEMENT SERVICES FOR  TOWN OF HAVANA

November 13, 2024

I. INTRODUCTION
Town of Havana is requesting written qualifications from qualified construction firms to provide Professional Continuing Contracts for Construction Management Services.

II. PROPOSAL INSTRUCTIONS AND GENERAL INFORMATION:
Proposal Submissions: Submit eight (8) sealed copies of a written proposal no later than 12:00 PM on 12/12/2024, labeled RFQ - Construction Management to:

Havana FL 32333

Submittals must be responsive to the requirements and questions of the Request for Qualification.
Reservations: Town of Havana reserves the right to reject any and all submittals, to negotiate changes in the scope of work or services to be provided, and to otherwise waive any technicalities or informalities.

Please respond by including but not limiting your response to the following:
1. Company name, location and length of time in business.
2. Verification of ability to do business in the State of Florida.
3. Demonstrate a capability to provide required Performance and Payment Bond and include name of bonding company.
4. Provide proof of commercial general liability insurance coverage (minimum $1 million per occurrence and $2 million aggregate) and statutory worker’s compensation coverage.
5. Provide a list of current, past, and pending litigation.
6. Provide a notarized Public Entity Crimes Statement, available at www.townofhavana.com.
7. Has your firm ever been terminated for cause or failed to complete a contract? If so, provide details and circumstances.
8. Explain any legal action the firm has experienced within the last 5-years or is currently experiencing.
9. Qualifications of staff to be utilized on this project with names, resumes, length of time with firm and previous clients served.
10. Names of previous clients within the past 5 years with phone numbers and contact person, with emphasis on similar project experience and examples.
11. Description of similar previous project experience, with emphasis on budget, final cost, time schedule, change orders, etc.
12. Examples of schedules for projects ranging in size and scope from $1 to $20 million with specific emphasis on methods for keeping project on time and in budget.
13. Company organizational chart with reference to this project.
14. Provide examples of records, reports, monitoring systems and information management systems you firm uses during both Pre-Construction and Construction Phases.
15. Provide an overview of the monthly reporting that you provide to Owners.
16. Identify the specific resources to be used on the project with respect to scheduling.
17. Describe your Site Safety Program.

The selection will be made based on the proposals received. Proposals will be opened immediately following the end of the submission period on 12/12/24 at 12:00 PM. There may be no presentations. Please do not contact Town of Havana regarding this project or RFQ.
